Ensure all Submissions Include:

• Name, address and email of the candidate

• A statement outlinging his/her personal contribution and that of others to the development of the device

• Copies of relevant publications relating to the device

• Letters from two professional accelerator physicist, engineers or laboratory administrative personnel who are familiar with the device and it's development
